Cullman City Schools Health Service Staff

Cullman City Schools require that an official medication authorization form be submitted by a student's parent/guardian before medication may be administered at school.  The form should be submitted to the appropriate school's nurse. 

The form can be downloaded, printed, completed, and returned to the appropriate school nurse.  Click the links below to access print options.
